A. OSAT and/or OPTE Institution
If you are registering for one or two OSATs or an OPTE, use the “ Institution Codes” table that follows to
indicate the Oklahoma institution where you completed the largest number of credit hours in the test area(s).
This institution may receive your OSAT and/or OPTE test score(s). If your teacher preparation or advanced
certification program was completed in another state, leave this question blank and complete “Out-of-State
Preparation” below. If you did not go through a teacher preparation or advanced certification
program for the certification area you are seeking with this assessment, leave this question blank.
B. OGET Institution (not required for those seeking a Paraprofessional Title I Credential)
If you are registering for the OGET, use the “ Institution Codes” table that follows to indicate the Oklahoma institution
where you completed the largest number of credit hours in general education. General education courses
are those in the areas of English, the humanities, social science, science, and mathematics that are typically
required by colleges to form the foundation of a college education. This institution may receive your OGET test
scores. If your general education preparation was completed in another state, leave this question blank and
complete “Out-of-State Preparation” below.
C. Out-of-State Preparation
If your teacher preparation or advanced certification program or your general education preparation was
completed outside the state of Oklahoma, identify the state in which you completed your teacher preparation or
advanced certification program or your general education preparation. If you are registering by mail, enter the
